Welcome to the C4U Registration page for Issuer Dedicated Sales Teams.

An Issuer Designated Sales Team is comprised of the Primary Account Holder, and Associated Sales Agents who are certified brokers employed by one of our partnering insurance companies offering plans on the Exchange.

In order to act on behalf of customers to enroll them in a health plan through the Exchange, Issuer Sales Team Agents must complete the training and certification program through Connect for Health Colorado annually, and have their broker licensure through DORA. Please complete all required fields on this form so that you can access and log into the new learning management system, C4U, to complete the required coursework for your certification.
